In most states, adding a father to a birth certificate after birth requires either a signed Voluntary Acknowledgement of Paternity (VAP) from both parents or, if either parent disputes it, an AABB-accredited legal DNA paternity test. If the mother was married to another man at the birth, a court order (based on a legal DNA test) is almost always required.
Uncontested cases: VAP is enough
